Scheduling

The Central Scheduling Line at The Centers for Dialysis Care welcomes all patient and referral source requests. CDC’s Central Scheduling works on obtaining an opening at the facility that best suits the patient’s needs. Once the slot has been obtained and accepted by the patient, the admission date is determined. The Central Scheduling team then works to obtain the required paperwork to facilitate the admission for the patient.

Central Scheduling will provide a list of Nephrologists for those that do not have a Kidney Doctor. Patients can either choose their own nephrologist or CDC can refer one to you. Once all paperwork has been obtained, it then gets forwarded to the admitting unit. If the referring hospital or physician has the ability to call the scheduling line in advance, it would be appreciated but not necessary.

Transportation should be provided by the patient/family for the first two weeks of dialysis. Patient’s transportation needs can be discussed when meeting with a Patient Service Representative upon the first visit to CDC. The Patient Service Representative will determine what type of transportation is available according to the insurance coverage a patient has. CDC offers Transportation Applications for those who need assistance with transportation.
