Transient Patients
CDC patients planning a trip or vacation
If you are a CDC patient planning a trip or vacation, we will assist you in setting up your dialysis in your destination city. Please speak to the Patient Service Representative (PSR) at your home unit to inform them of your travel plans. You may also request a trip / vacation form from your home unit. Once the form has been completed it should be given to the PSR for that facility. The PSR will contact a dialysis unit I the area in which you are traveling and arrange your dialysis treatments. If you have any questions, please contact your PSR.
Non-CDC patients needing to dialyze at CDC
If you are a transient patient from a non-CDC unit wishing to dialyze at CDC, please contact the Transient Line at 216-732-3755. You will need to provide the following information to initiate the plans for your treatments:
- Name
- Telephone number
- Name of Home Dialysis Unit
- Name and phone number of Social Worker
- How many hours do you dialyze? How many treatments needed
- Insurance information
CDC accepts most all insurances. Upon verification of your insurance coverage, if it is noted that you do not have 100% coverage you will be asked for a co-insurance pre-payment to cover your treatments. This payment can be made by cash, check or credit card in advance to the treatments. If you have any questions please contact the Transient Line at 216-732-3755.
